    You might want to try eHealthInsurance.com over at Befree. They pay from $7 to $40 depending on the different applications. And Phillip is a great Affiliate Manager!

    They don't convert real well for me only because I am a shopping based site. None of the financial/credit card/insurance deals do well for me.
    My visitors just want to shop.

    Insweb will pay you $2 and $11 for auto leads and $30 or more for life insurance. At this time we don't do health. A good mix is auto insurance and auto loans.
    Insweb is the #1 site according to Kiplingers, Yahoo Magazine, etc. try us out.
