Can you please give me your opinion on Lead Gen programs for Insurance?

Which is better:
1) A lead gen program that only requires name, address, and email. Could possibly have customer pay $1 to qualify lead. Merchant pays less per lead but qiucker app; less qualified. Concern here is hi fraud rate - any anecdotes here? Benefit to affiliate is possibly higher conversion/more leads but pays less.

or

2) Full app, with more questions (i.e. medical, safety record), full name, address, email, and customer possibly pays a small app fee, say $1 (or not at all) to qualify traffic. Concern here is that takes longer for customer to fill out app, and could leave without completion. Benifit of course to affiliate is delivering better quality traffic but less qualified and pays more.
