We had an issue with Quixsurf recently on CJ, in which their toolbar seemed to have been installed on Call Center employees' machines without their knowing. Subsequently orders were attributed to Quixsurf, when call center people went to our site and placed orders, because they'd been cookied by Quixsurf. We caught them when we realized a bunch of affiliate sales were coming from internal IPs...
